The Alters casts the player as Jan Dolski, the lone survivor of an expedition that crashed on a hostile planet. To handle tasks beyond one person, he uses a substance called Rapidium to create alternative versions of himself, the Alters, each shaped by a different crucial decision from his past. These variants bring their own skills and needs, and managing relations with them and trying to influence their choices drives a branching narrative with moral dilemmas and multiple endings. The mobile base must be kept running with metals and organics gathered on the surface, and it can be expanded with modules such as a research lab or a social room. Expeditions outside the safe haven expose Jan and his Alters to deadly anomalies and the sun's radiation, so the day and night cycle has to be taken into account when planning activities. The deluxe edition includes the base game, costumes, the original soundtrack and the Last Variable expansion, which continues the storyline in which one version of Jan remains on the planet to uncover its secrets.

Beyond the base game, the Deluxe Edition adds an exclusive in-game outfit, the original soundtrack and the Last Variable expansion. The expansion is paid content that becomes available after release and continues the storyline of the Jan who stays on the planet; average playtime is roughly 20 hours.
